The Licensed Practical Nurse is a key member of the care delivery team and communicates the needs of the patient and the team; collaborates with the multidisciplinary team, and delegates appropriately to the other members of the team. HILDEBRANDT HOSPICE CENTERHildebrandt Hospice Care Center is an 11-bed facility at 2652 Ridgeway Avenue that embodies the promise of comfort and dignity by providing end of life care. Nurses, Evaluators and Hospice Aides work directly with patients and support their families through the loss of a loved one. We partner with hospitals and health facilities to provide end to end continuity of patient care. Provides routine nursing services for patients as directed by the RN. Conducts and documents a thorough observation of each patient’s medical status upon admission and throughout the patient’s course of treatment. Participates as a member of the care delivery team, assists in the implementation of the care plan for each patient, and models exceptional teamwork to other members of the team. Planning & Communication. Provides regular patient status updates to the appropriate personnel. Assists in the delivery high quality, safe care through effective communication, exceptional collaboration and attention to the patient’s condition. Assists in the implementation of the patient’s treatment plan under the direction of the RN.Electronic Health Record (EHR) Management. Completes required documentation in an accurate and timely manner. Complies with the evaluation, treatment, and documentation guidelines of the department.Compliance. Acts in compliance with hospital, regulatory, and professional standards and policies.EDUCATION:LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. It may be modified in the future and eligible for additional pay components.
