Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team. As a RN Case Manager, you will play a critical role in ensuring the highest quality of care for our patients and their families. Your expertise and dedication will be instrumental in making a meaningful difference in the lives of those we serve.

Responsibilities
  • Conduct thorough initial client assessments and develop comprehensive care plans that address the unique needs of each patient.
  • Provide hands-on nursing care and education to patients and their families, promoting independence and self-advocacy.
  • Maintain open and effective communication with patients, families, and the healthcare team to ensure seamless care coordination.
  • Manage patient care coordination for your assigned caseload, ensuring timely and efficient delivery of services.
  • Participate in interdisciplinary team meetings to develop and implement comprehensive care plans that meet the complex needs of our patients.
Qualifications
  • Licensed as a Registered Nurse (RN) in the state where you will be working.
  • Proficiency in Electronic Medical Records (EMR) software.
  • Excellent planning, organizing, and communication skills, with the ability to work effectively in a fast-paced environment.
  • Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards of care.
  • Valid Driver's License and reliable vehicle with current insurance.
