Job Description:
The Total Joint Program RN Coordinator is responsible for the development, coordination, and maintenance of the care continuum for total joint replacement patients. This role provides leadership across all stages of patient care, from pre-surgery through one year post-op, involving Pre-Admission Testing, Perioperative Services, inpatient units, post-acute care, and other relevant entities within the orthopedic service line. The Coordinator ensures adherence to evidence-based practices for orthopedic patients and assists in the planning, development, and implementation of program goals and operations. The role also oversees the ongoing disease-specific certification process to continually enhance care for the orthopedic patient population.
Education:
* Bachelor of Science in Nursing - Required
* Master of Science in Nursing - Preferred
Experience:
* Healthcare - Orthopedic Surgery - Required
* Healthcare - Case Management - Required
Certifications/Licenses
* RN Registered Nursing - California Board of Nursing - Required
* BLS Basic Life Support - American Heart Association - Required
* CCM Certified Case Manager - CCMC Commission for Case Manager Certification -or -
* ACM Accredited Case Manager - ACMA American Case Management Association - Required
* ONC Orthopedic Nurse Certified - ONCB Orthopedic Nurses Certification Board - Preferred
Skills:
* Proficiency in using electronic medical records (EMR) systems
* Knowledge of current best practices and guidelines in orthopedic care
* Ability to work collaboratively and build consensus across multiple departments to achieve desired outcomes
* Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members
* Strong organizational and time management skills to handle multiple priorities and task
* Capacity to lead quality improvement initiatives and analyze data for performance improvement
* Ability to adapt to changes in healthcare regulations and practices
* Ability to handle confidential information with discretion
* Strong critical thinking skills to evaluate and improve clinical practices
* Strong written and verbal communication skills with a focus on customer service.
* Knowledgeable about Medicare and commercial contracts for acute and post-acute care.
Knowledge of CMS and TJC regulations preferred
Work Shift:
Exempt Days (Mon-Fri) (United States of America)
Pay Range: $84.21 - $113.47Hourly
Offer amounts are based on demonstrated/relevant experience and/or licensure.
Note: Positions at JMH which are exempt (not eligible for overtime) under the level of Manager are listed as hourly for compensation purposes on this posting. The work shift will contain the word 'exempt' on it.
