Intake and orientation Case management, referrals, care coordination and connection to community providers and services Provides basic crisis responses and client support pre and post admission Group facilitation as permitted under the required OASAS Scope of Practice supervision and oversightProvides psychiatric and addiction educationResponsible for reporting and record keeping required for programs and outside stakeholders (OASAS, legal, county, grants etc.)Makes collateral calls, obtaining consentsEngaging clients during intake and pre-admission via telephonic, video and in person methods.Preparing and obtaining toxicology screens (observing)Participates as a contributing member of an interdisciplinary team in coordination and delivery of patient care. PAY RANGE:$18.25 - $25.00CITY:RochesterPOSTAL CODE:14626The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
