POSITION: Case Manager DEPARTMENT: Youth Services

WORK SCHEDULE: 1:00 p.m. - 9:00 p.m., (Tuesday through Saturday for Residential; as scheduled for Day Reporting)

SALARY: $1,910.04 Biweekly

DUTIES:

In the absence of a therapist, complete a Brief Assessment within 24 hours on all new admissions and record residents in Informer.

Arrange contact, engagement and supportive services with youth and families; communicating regularly with youth and families to answer questions and provide coordination with regard to care, treatment plans and transition planning.

Plan, implement, and attend Family Engagement Activities; ensure appropriate coverage if unable to attend.

Work with Staff Sponsors and Supervisors to ensure Individualized Treatment Plan is completed with parents, youth and other supportive adults within contract guidelines and expectations.

Facilitate evidence-based groups for adolescent males and females, ages 11 through 18 years old, in compliance with Youth Service's Standards of Care.

Complete required Progress Reports, Transition Plans, Discharge Summaries, and daily documentation of services provided to youth as assigned.

Ensures youth in care have basic needs met such as clothing, haircuts, hygiene, school supplies, work needs, etc.

Participate in assigned Staffings and Treatment Teams and other supervision as designated.

Complete all necessary admission documents for youth; ensuring signatures and complete records are obtained.

Coordinate transportation for youth using community resources and services; ensure transportation log is accurate and transporters are assigned accurately.

Ensure Independent Living (IL) Skills development for youth occurs in addition to other specialized skill development needs as identified.:

  • IL skills for residential youth include oversight of weekly IL groups to ensure that topics are beneficial, relevant, that activities are experiential in nature, and that groups are taking place in the community when appropriate. Additionally, providing oversight that all IL expectations are met within the stated timelines for youth 16 and older and communicated to all members of the Treatment Team. This includes but is not limited to completion of the Casey Life Skills assessment, an IL Learning Plan and weekly individual and group IL notes. Tasks also include ensuring that the family completes the Casey Life Skills assessment regarding the youth and participates in the development of the IL Learning Plan. Also maintains a monthly IL spreadsheet and enters data into the National Youth in Transition Database (NYTD) per state requirements.

Ensure compliance with practices of DBT, Five Domains of Wellbeing, Motivational Interviewing, Interactive Journaling, Youth Competency Assessment and other evidence-based programming.

Assists Deputy and Executive Directors and Therapist in projects related to Outcome Management through IARCA as well as other specialized skill development needs. This includes assistance with post-discharge program follow up / aftercare.

Plan and implement Behavioral Contracts.

Assist with coverage of Case Conferences and other meetings for youth in conjunction with Therapist and Deputy Director.

Coordinate employment and extra-curricular responsibilities for youth as applicable.

Provide court testimony as needed.

Perform related duties as assigned.

JOB REQUIREMENTS:

Bachelor's Degree in Counseling, Law and Society, Human Services, Social Work, or related field.

Previous experience with treating children or adolescents and case management.

Ability to work autonomously or with moderate direct supervision.

Ability to organize and prioritize time and work responsibilities.

Attention to detail.

Knowledge of services and resources for children and adolescents.

Thorough knowledge of all state and local rules, regulations and policies related to residential and emergency shelter care.

The ability to assess, counsel, and provide interventions for high-risk youths and their families.

Ability to be responsive to diversity.

Ability to work non-traditional schedule and be available to meet at times when family is available.

Ability to communicate orally and in writing with Clinical Staff and Youth Development Specialists, Workers, and Supervisors, Day Reporting Staff, youth and their families, placing agencies, and the public, including being sensitive to professional ethics, gender, cultural diversities and disabilities.

Ability to understand, memorize, retain, and carry out written or oral instructions and present findings in oral or written form.

Working knowledge of standard office procedures and computer software programs used by the Prosecuting Attorney's Office, with ability to apply such knowledge to a variety of interrelated processes, tasks, and operations.

Ability to maintain confidentiality of all department records and information.

Ability to follow all ethical guidelines.

DIFFICULTY OF WORK:

Incumbent performs the development, supervision, and implementation of treatment plans, through a variety of modalities for an at-risk adolescent population.

Residents served may suffer from significant emotional or behavioral problems.

RESPONSIBILITIES:

Incumbent assures ethical, confidential and sound practices for the supervision and care of diverse children and adolescents.

PERSONAL WORK RELATIONSHIP:

Incumbent maintains daily communication with the clinical staff and Youth Development Specialists, Workers and Supervisors in coordinating care with youth and their families.

Case Manager reports directly to the Deputy Director (for Day Reporting) and to the Executive Director (for Residential) programming.

PHYSICAL EFFORT AND WORK ENVIRONMENT:

Incumbent works in a residential child care institution and must be able to sit or stand for long periods of time, and perform physical interventions as needed, related to medical or behavioral needs.
