Job Title: Case Manager

Omni Visions is seeking a dedicated and compassionate Case Manager to join our team. As a key member of our Family Services team, you will play a vital role in providing support and guidance to individuals and families in need.

Job Summary:

The Case Manager will be responsible for assessing, advocating, linking, and coordinating community-based services for clients and families. This will involve working closely with clients, community agencies, and other stakeholders to ensure that clients receive the support and resources they need to achieve their goals.

Key Responsibilities:
  • Conduct comprehensive assessments of clients' needs and develop individualized service plans
  • Provide counseling and therapeutic services to clients and their families
  • Link clients with community-based services and resources
  • Advocate for clients' rights and interests
  • Monitor and evaluate the effectiveness of service plans and make adjustments as needed
Requirements:
  • Bachelor's degree in social sciences or related field
  • 1 year of relevant professional experience working with families and children
  • Training in crisis prevention initiatives and universal precautions
  • Valid driver's license and reliable transportation
