Transitions is seeking a Patient Care Manager with hospice and/or health care experience to join our growing family The ideal candidate will assists the Administrator/Director of Nursing with adherence to the policy and procedure manuals that establish and support the highest possible quality of care to patients and families. Local Travel is Necessary.

The Patient Care Manager (PCM) is a member of the leadership team that works collaboratively with the Administrator/Director of Nursing with all patient care activities to ensure the delivery of safe and efficient patient care which adheres to relevant standards of accepted nursing, regulatory, medical practice, and payer source criteria. The PCM works to offer support and education to team members that is consistent with the identified needs based on a review of the documentation, compliance audits and reported areas of weakness.
  • Responsible for team compliance with policies and procedures as well as compliance with local, state, and federal regulatory requirements governing the practice of the assigned team.
  • Reviews admission documentation for appropriateness of care and consistency of documentation in coordination with the Director of Nursing.
  • Assists the Administrator/Director of Nursing with adherence to the policy and procedure manuals that establish and support the highest possible quality of care to patients and families.
  • Serves as a contributing member for Interdisciplinary Group, Quality Assurance Performance Improvement initiatives, including completion of chart audits and strategic agency planning.
  • Ensures that the plan of care meets all regulatory requirements including appropriate personalization based on the patient's initial and ongoing assessment.
  • Reviews hospice team death, discharge, transfer, revocation and level of care change documentation ensuring all regulatory components are met.
  • Ensures all appropriate external documentation is obtained including but not limited to: History & physicals, diagnostic imaging, lab work (as available), power of attorney (POA) paperwork, and POLST paperwork.
  • Works collaboratively with the field staff to assure documentation is complete and consistent with care and reflects legal requirements.
  • Assists in ensuring the team is producing high quality care that is accurately reflected in all documentation.
  • Assists in routine approvals of orders, plan of care updates and daily changes ensuring Transitions standards are exceeded in every encounter.
  • Works collaboratively with the teams to provide quality improvement activities utilizing continuous quality improvement principles and methodologies for their designated area.
  • Compiles, trends, and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes and patient satisfaction.
  • Ensures initial and ongoing orientation for assigned team and collaborates with Administrator/Director of Nursing on evaluation of assigned team members.
  • Ensures compliance with regularly scheduled continuing education for assigned teams.
  • Assists Administrator/Director of Nursing with oversight of care provided by staff that promotes the health and safety of the patient.
  • Provides guidance to the hospice team based on the clinical needs of the patient observed in the documentation.
  • Provides patient care according to plan of care, as ordered by the physician.
  • Ensures compliance with CMS submission requirement (HIS)
  • Maintains current knowledge related to Medicare Conditions of Participation and accrediting organization standards to ensure that the organization is compliant with state, federal and accreditation guidelines, as appropriate.
  • Additional audits as warranted to ensure compliance with state and federal regulations.
  • Participate in staff meetings, department meetings, team meetings, briefing, in-services, and other related activities as needed
  • Assure for compliance with local, state, and federal laws, Medicare regulations and established company policies and procedures.
  • Regular attendance is to be maintained
  • Adherence to a code of conduct conducive with Transitions is expected
  • Meet or exceed delivery of Company service standards in a consistent fashion
  • Interact with all staff in a positive and motivational fashion supporting the Company mission
  • Conduct all business activities in a professional and ethical manner
Requirements
  • Graduate of an accredited school of nursing. BSN preferred.
  • Valid license as a Registered Nurse in the state of employment.
  • Certification as a Certified Hospice and Palliative Care Nurse (CHPN) preferred
  • Current Basic Life Support Certification
  • Valid Driver's License & active car insurance
  • Minimum of three (3) years of hospice experience required
  • Minimum one-year of field-based current nursing experience is recommended.
  • Good written and verbal communication skills.
  • Sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death.
  • Excellent time management and organizational skills are essential.
  • Critical thinking skills and ability to solve problems.
  • Strong computer skills are required, such as, email, and prior experience working with electronic medical records.
