What You Will Do:
- Utilize approved assessments to identify clients/members needs and family needs; develop initial and ongoing clinical plan of care
- Perform and maintain effective care management for assigned caseload of clients/members
- Provide supportive counseling and ongoing mental health services
- Collaborate and refer to appropriate agencies as required; Address any client/member concerns to ensure satisfaction with overall services provided and use motivational interviewing techniques to foster behavioral changes
