Per Diem Home Health Physical Therapist

Become a part of our caring community and join us in prioritizing health as a Per Diem Home Health Physical Therapist. In this role, you will have the opportunity to plan and administer prescribed skilled physical therapy treatment and training for patients. You will work with a team to develop and implement conditioning and rehabilitation programs, monitor patient progress, and provide valuable input to the patient's physician and clinical team members. Additionally, you will play a crucial role in supervising and evaluating staff performance and ensuring accurate documentation of patient care. If you possess a degree from an accredited Physical Therapy Program and have a minimum of one year of physical therapy experience, we encourage you to apply and join our team in making a positive impact on the lives of our patients.
Required Experience/Skills:
- Degree from an accredited Physical Therapy Program (approved by the APTA)
- Minimum of one year physical therapy experience preferred
- Current and unrestricted Physical Therapy license
- Current CPR certification
- Strong organizational and communication skills
- A valid driver's license, auto insurance, and reliable transportation are required
