Job Summary:
We are seeking a dedicated Physical Therapist to join our team at Trinity Health PACE. As a Physical Therapist, you will play a pivotal role in delivering clinical evaluations and treatment in acute, restorative, or maintenance settings across our Day Center, Nursing Home, or in-home environments.
Key Responsibilities:
- Conduct comprehensive physical therapy assessments and develop individualized care plans.
- Provide therapeutic interventions to participants with diverse disabilities, promoting maximum functional independence.
- Collaborate with interdisciplinary teams and assist in managing durable medical equipment needs.
- Train staff in safe body mechanics and use of therapy equipment.
- Maintain high standards of documentation, safety, and quality assurance.
Requirements:
- Bachelor's degree in Physical Therapy from an accredited program.
- Active, unencumbered physical therapy license in the state of North Carolina.
- At least one year of experience working with frail or elderly populations; less experience requires PACE-specific training.
- CPR and First Aid certification may be required.
- Demonstrated ability to work independently, manage conflict, and communicate effectively with diverse groups.
