Position Description
The Engagement Support Specialist works closely with the clients to implement the goals identified during the intake, assessment, reassessment process, and care plan. The Engagement Specialist will collaborate with the supervisory team to provide coverage for caseload(s) needing assistance; for example, when the assigned Care Manager is out sick, on vacation, on medical leave, or there are any vacancies in the program. As a direct care provider, the Engagement Support Specialist will be responsible for helping clients obtain the services/care identified in the care plan and assessment. In addition, the person in this role will assist the clients by gathering information to complete assessments and reassessments. This person will need to conduct home visits/field work, maintain contact with the client’s extended family/informal support networks for care coordination, accompany clients to/from medical appointments, and assist clients in obtaining entitlements and other services.

The part-time position requires a schedule of three days a week, totaling 20 hours. The Engagement Support Specialist will work on Mondays from 9 a.m. to 5 p.m. and on Tuesdays and Wednesdays from 9 a.m. to 4 p.m. Alternatively, they can work on Wednesdays from 9 a.m. to 5 p.m. and Thursdays and Fridays from 9 a.m. to 4 p.m. The final schedule will be confirmed by the hiring manager at the time of hire.

Essential Job Functions

The following duties are mandatory requirements of the job:
  • Gather information to complete assessments and reassessments.
  • Help clients obtain services as identified on the care plan/assessment.
  • Conduct home visits and community follow-ups to work in the community.
  • Evaluate home environment through home visits.
  • Maintain contact with the client’s extended family and informal support networks.
  • Escort clients to/from medical appointments.
  • Outreach via phone when the client has yet to keep necessary appointments.
  • Outreach via phone between visits to check on self-care, medication fills, treatment plan, schedules visits, tests/follow-up.
  • Identify the primary care physician or psychiatrist to provide care coordination as needed.
  • Assist clients in obtaining entitlements.
  • Assist in crisis intervention.
Other Responsibilities

The following duties are to be performed as assigned by the supervisor:
  • Identify quality of care issues and refers to appropriate departments/services.
  • Provide interpreter services as required.
  • Attend training and meetings as needed to comply with the training requirements for the program staff.
  • Perform other related duties as required.
Minimum qualifications

Education:
  • Associate Degree in Social Service preferred or High School Diploma or equivalent.
Special skills and knowledge
  • Computer skills.
  • Knowledge of community resources and their financial requirements.
  • Good oral and written communication skills.
  • Good organizational skills.
  • Good interpersonal skills.
  • Good time management skills.
  • Able to speak, read, and writes English to the extent required by the position.
  • Bilingual Spanish required.

The ideal candidate must have excellent organization and time management skills, good written and communication skills, and be a team player with a positive approach to working with others.
