We are currently seeking an empathetic and dedicated Intake Coordinator to play a vital role in our men's drug and alcohol relapse prevention program. As the Intake Coordinator, you will be the first point of contact for individuals seeking admission to our program. Working under the supervision of the program manager, you will conduct thorough assessments and provide support to clients battling substance abuse. Your responsibilities will include facilitating access to essential services, conducting assessments, evaluating applicants who would be a good fit for the program, and crisis intervention strategies as needed. In addition to serving all program residents, including veterans, you will represent The Salvation Army at community meetings and activities, advocating for our clients' needs and promoting our mission of hope and healing We are seeking candidates with a compassionate approach, strong communication skills, and a commitment to empowering individuals to achieve lasting change in their lives. Duties and Responsibilities: Will work closely with clients seeking entrance into substance abuse and transitional housing program to determine need and eligibility. Responsible for daily intake data entry of client information into Wellsky data base and HMIS. Responsible for client screening/assessment, intake and orientation of new clients into the Harbor Light program. Maintains communication with Case Managers, Probation Officers, and perspective clients in the community. Works closely with the Case Managers from the Veterans Affairs Dept to manage logistics for incoming Veteran clients. Manages a list of perspective clients and makes referrals to other resources as necessary. Under the supervision of the Program Manager, is responsible for developing a system for screening, enrolling and tracking all funds throughout the clients stay in the program. Will work closely with funding agencies to meet intake, assessment and reporting requirements. Follow up and resolve any issues regarding client enrollment. Will handle all paperwork associated with Program Fees & Food Stamp programs to make certain contract guidelines are being followed. Prepare reporting information for contracts for Dept of Human Services Food stamps. Attend and participate in assigned staff meetings and work as a team with other staff. The individual must show respect for other employees, associates, clients, and the goals and objectives of The Salvation Army. Carry out other duties as assigned by the Progr m Manager and Program Administrator Perform other duties as assigned Qualification and Education Requirements: Heart and passion for The Salvation Army's mission. Natural ability to reflect and model the high standards of The Salvation Army as one of the world's most distinguished human services charitable organization Qualifications: Education and Work Experience High school diploma or GED with some College required. Education and/or work experience commensurate with the duties and responsibilities required including understanding alcohol and/or substance abuse. CACI preferred. Education and/or work experience in human services helpful. Valid Driver License required. The individual must have ability to speak, read and write the English language with a professional level skill is required. Strong data entry and organizational skills. Familiarity with HMIS a plus. Qualifications: Knowledge, Skills and Abilities Required Familiar with government and agency grants, able to read and interpret the needs of funding agencies. Ability to work with funding agencies for enrollment of clients into programs including, but not limited to food stamps (SNAP).
