Your Mission:- Deliver exceptional primary care to a dedicated panel of patients, focusing on their unique health goals.
- Champion the 'treat in place' model, ensuring advanced care planning is seamless and tailored to each individual.
- Conduct thorough assessments, create personalized care plans, and coordinate with care teams to ensure the best possible outcomes.
- Play a key role in clinical operations such as prior authorizations, medication reviews, and closing quality care gaps (HEDIS and STARs).
- Act as a trusted advisor to patients, families, and care teams, ensuring everyone is informed and aligned on care needs and goals.

Unrestricted state licensure as an Advanced Practice Nurse and national board certification.

Your Mission: -
6+ months of experience as an Advanced Practice Provider (preferably with geriatric, hospice, or palliative care populations).

Strong clinical skills and experience in treating chronically ill patients, with a preference for nursing home environments.

Tech-savvy with excellent communication skills to collaborate effectively with patients and healthcare teams.
