This is done with an emphasis on early assessment of discharge needs as well as communication among the healthcare team members and the family/care givers. The POE/ED CM is responsible for collaborating with the admitting teams to ensure the correct initial admission status (INPT or OBS) at the time of admission using Interqual criteria. Qualifications: Education: Bachelor of Science in Nursing required. Certification/License: Current Massachusetts licensure required. CCM preferred. Experience:# 3-5 years of hospital-based case management with emergency department experience preferred Minimum of three years of medical, surgical, ICU or ED nursing experience required Demonstrated ability to make sound judgments with knowledge of current medical care technology and ancillary supports. Statement of Purpose
