As an RN Case Manager, you will conduct in-home nursing visits for clients who are former Nuclear Weapons Workers suffering from chronic or terminal illnesses caused by exposure to toxic substances during their government employment. Expect to provide care through weekly visits that typically last between 1-2 hours. Your responsibilities will include performing physical assessments, managing home safety with medical equipment and supplies, organizing weekly medication planners, and overseeing skilled and non-skilled caregivers.
Key Responsibilities:
- Provide comprehensive case management for assigned clients while leading a team of RNs, LPNs, and caregivers in delivering quality care.
- Collaborate with the regional director and team members to identify areas for improvement in service delivery.
- Plan, implement and evaluate individual client care, conducting care conferences as required.
- Serve as a clinical resource to ensure that clients' home care needs are effectively met.
- Assist staffing coordinators to ensure adequate staffing that matches client needs and care complexity.
- Follow defined nursing delegation guidelines to ensure best practices are adhered to.
- Participate in recruitment efforts, staff training, and performance evaluations as necessary.
- Engage in agency performance improvement projects.
- Maintain accurate clinical documentation and review quality of care deliveries.
- Conduct supervisory visits as needed, focusing on problem resolution in care delivery.
- Contribute to client outreach and marketing as requested.
Qualifications:
- Graduate of a state-approved nursing program.
- BSN degree preferred.
- Current, unrestricted RN license in the state of practice.
- A minimum of two years nursing experience, with at least one year in home care or a similar field.
- Preferred supervisory or case management experience of one year.
- Current CPR certification.
