Meridian, United States St. Luke's Health System Full time
Overview

At St. Luke’s, our dedicated team of RNs strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.

St. Luke’s Treasure Valley home health team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home.  From newborn to seniors, we offer the latest home health techniques and technology to people at all ages and stages of life.  Dedicated to the continuum of care as patients transition from inpatient to home to outpatient settings for care.  Our home-based services team (NP, RN, MA, MSW) also assists with oversighting transitions.  Close working relationship with St. Luke’s Hospice team.

You will support our Home Health and Hospital at Home services.

As a Home Care RN, you will provide comprehensive care to patients with a new diagnosis, medications, or have been discharged from ER, require respiratory treatment or wound care. During home visits, you will ensure patients are progressing with their care plan, provide education related to their disease process, and assess physical and psychological needs to improve quality of life. When it comes time to discharge, you will coordinate resources and educate patient and patient's family.

Why Home Health"When the time comes to discharge a patient, it can be difficult because you created a special bond with them and their family and caregivers, as they allowed us the honor of coming into their homes to walk side by side with them.  This is the moment I love to tell them how rewarding it has been, as they are now able to "graduate" from home care and begin living their best lives"~St. Luke's Home Health RN
