POSITION TITLE: Pathway of Hope Case Manager

LOCATION/DEPT: Rockford

REPORTS TO: Director of Social Services

OUTCOMES

Provide compassionate, professional, and holistic social service assistance to clients regardless of their background. Services are provided from a visionary perspective grounded in the mission of The Salvation Army and executed in a compassionate manner. Interaction with Salvation Army employees, other agencies, and the community result in a collaborative effort to meet client needs. Client contact reflects the mission and teaching of The Salvation Army.

ESSENTIAL DUTIES AND RESPONSIBILITIES

PATHWAY OF HOPE

Provide direct service, community resource development, and casework consultation to ensure the full implementation of the Pathway of Hope model within the assigned Corps cluster. Many of these responsibilities will overlap with the social worker duties outlined above.

Program Implementation

  1. Provides intake and assessment services as defined by the Pathway of Hope curriculum. Assess needs and verify eligibility.
  2. Determine appropriate assistance grant to be awarded, verify information given, and initiate procedures for assistance.
  3. Works with at least 15 families over the course of one year and work maintaining a caseload of 8 families at any given time. Interventions include engagement of new clients, completions of comprehensive assessments and action plans, referrals to services designed to address identified goals, and plan developments for each family addressing transitional needs.
  4. Enters client data and reporting of information into the SIMS system that supports local, national, and territorial POH program outcomes.
  5. Maintains case files for each client that includes all assessments, action plans, case notes, graduation plans.
  6. Participates with POH planning and program development at assigned sites.
  7. Participates in scheduled cluster meetings, conference calls and individual consultation/supervision meetings with the Divisional Pathway of Hope Regional Coordinator.
  8. Participates in Divisional or Territorial educational forums and webinars designed to facilitate understanding of a wide range of topics including motivational interviewing, cultural competency, strength-based case management, stages of change model, smart goals, time management, advocacy, etc.
  9. Maintains working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.

Pathway of Hope Program Evaluation and Outcomes Measurements

  1. Ensure the accuracy of data entry into the database.
  2. Supports the POH implementation plan as requested and/or assigned.
  3. Serves on committees related to case management, emergency assistance and building sufficiency, as requested, or appointed.

Other Duties

  1. Distribute in-kind or financial aid according to resources available.
  2. Maintain a monthly count of services rendered and submit for inclusion in monthly statistical report.
  3. Make referrals based upon need and as part of a total service plan, coordinating Specific arrangements to apply for additional resources when appropriate.
  4. Contact outside agencies regarding status of clients, obtain supplementary information, or verify the ability of the agency to meet the client's needs through community resources.
  5. Participate at Social Service provider network meetings and in-house meetings
  6. Attend all staff meetings.
  7. Answer telephone calls when needed.
  8. All other duties as assigned.

REPORTING RELATIONSHIPS

This position reports to the Director of Social Services.

In contacts related to this position's duties, this individual acts as a representative of the Army and its mission.

PERFORMANCE MEASUREMENTS

This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of accomplishing assigned goals.

EDUCATION/EXPERIENCE

  • Bachelor's degree preferred. A social work major is preferred but other human service fields will be considered.
  • Requires two years' experience in case management.
  • Central Territory Caseworker Certification Program completion is required upon hire.
  • Requires two years' experience in case management.

COMPENTENCIES

  • Bi-lingual (Spanish) preferred.
  • Must be able to work as a team member.

POSITION LIMITATIONS

  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Director of Social Services informed on all critical issues relating to his/her area of responsibility.
  • This individual will adhere to all Army policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • This position is required to do light physical work.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and computer.
  • The work environment for this position includes an office environment with a low noise level.
