RN Case Manager- Oswego Hospital

Collaborates with multi-disciplinary teams to assure high-quality, cost-effective care for patients. Coordinates individual patient care throughout the continuum of services.

Effectively interacts with physicians, patients, and health care team members while maintaining high professional standards. Performs the UR/QA function for case load. Uses advanced analytical judgement to find creative solutions in a variety of situations. Assures compliance with the Mission Statement of Oswego Hospital by actively supporting the goals for the department and hospital.

Graduate of an approved RN program, and current licensure in NYS required. Bachelor degree in social work or related health care profession preferred. Minimum of 5 years clinical experience required. Demonstrated competence in discharge planning, social work, utilization review, quality improvement, general health care financial management, and computer skills preferred.

Pay Range: $35.00-$41.20/hour

Employee salary is based on commensurate experience and other qualifications.

In addition, Oswego Health includes the Oswego Health captive professional corporation, Physician Care P.C., providing physician services in orthopedics, cardiology, ENT, gastroenterology, breast care, audiology, general surgery, bariatrics, and primary care.
