BASIC PURPOSE OF THE JOB

Responsible for Social Work departmental management, assists with program development and coordination, and general operations of Social Work Services.

REPORTS TO

Director of Care Coordination

JOB REQUIREMENTS

Supervisory Responsibilities: YES

Minimum Education: Masters

Degree: Social Work

License/Certification Required: Tennessee L.C.S.W. required.

Minimum Work Experience: 6+ years post-Masters clinical and/or administrative experience required. Prefer leadership experience. Post-Masters experience in hospital setting required. Pediatric experience preferred. Experience working in a case management/social work model.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
  • Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team and community agencies.
  • Outstanding communication and negotiation skills.
  • Knowledgeable of federal, state and accreditation rules and regulations.
DUTIES AND RESPONSIBILITIES

Assists the Director of Care Coordination with the following:
  • Partners with local colleges and universities to create Internships for Social Work graduate students.
  • Adheres to NASW Code of Ethics and confidentiality parameters established by Social Work licensure laws in the state of Tennessee. Promotes hospital wide awareness of professional ethics regarding confidentiality in all circumstances.
  • Creates and maintains positive working relationships with community agencies. Serves as patient/family advocate and liaison with medical staff, health care team and resource agencies. Advocates for appropriate health care utilization by patients and families.
  • Coordinates department functions with other hospital departments and maintains good employee relations, interdepartmental relations, and public relations.
  • Maintains professional growth and development. Attends external meetings to remain aware of new programs and systems related to the Social Work field. Effectively communicates organization, departmental, and regulatory agency information to staff.
  • Provides leadership and guidance. Encourages staff development and growth.
  • Plans, coordinates & evaluates the Social Work Service for hospital campus, works to improve &/or maintain quality of service.
  • Assists with the development, implementation & evaluation of social work services within the framework of long- and short-term goals. Representing the department & hospital within the community.
  • Assists with creating and implementing policies & procedures in relation to the department and organization.
  • Acts as an ambassador for Children's Hospital while at work & at Children's Hospital sponsored events.
  • Demonstrates accountability and responsible use of Children's Hospital and Department time and resources.
  • Manages the medical Social Work staff:
    • Assists with hiring (Recruit, interview and orient new social worker) to include Hospital/MOB/PRN Staff Interpreters and Language assessment for offsite candidates
    • Coordinates on-boarding process for medical social workers.
    • Manages scheduling and payroll for Social Work staff: coordinates request for time off and & ensures coverage
  • Completes Annual performance evaluations for social work, evaluates performance and makes recommendations for personnel actions such as merit increases, promotions, and corrective action.
    • Establishes and maintains regular QI Review schedule
    • Supports staff's daily operations
- Social work assessments and direct patient care as needed, following the duties and responsibilities of a medical social work.
  • Hospital related responsibilities
    • A resource & consultant to staff & students regarding Best Practice Standards for social work services
    • Provides Cultural Diversity training & in-service education for ETCH staff.
    • Social Work representative on hospital committees, general orientation, CME presentations, etc.
    • Conflict resolution/Root Cause Analysis/Safety Events
    • Updates and maintains orientation materials
  • Reviews department performance, effects needed changes to improve services and ensure compliance with both hospital guidelines and regulatory requirements.
  • Ensures that the department operates within allocated funds. Applies cost-effective measures to select and purchase equipment and supplies via capital budget process.
  • Manages and utilizes hospital resources and time respectfully and accountably.
  • Willingly accepts any other assignment that may be requested.
PHYSICAL REQUIREMENTS-
  • Note: Reasonable accommodations may be made for individuals with disabilities to perform the essential functions of this position.
    • Medium lifting, pushing and pulling is required for 20-50 lbs occasionally, 10-20 pounds frequently and 10 lbs constantly to move objects. Sudden emergency situations have the potential for exposure for lifting or moving of up to 100 lbs. Frequent bending, walking, sitting, squatting, reaching, and standing are required. Keyboard/computer use and/or repetitive motions may be required.
