Job Summary:
Responsible to triage patient care both in the Clinic and by telephone. The LPN will obtain appropriate chief complaint from the patient/parent, take a brief medical history, obtain vital signs and growth parameters, perform basic laboratory procedures, assist provider as needed and direct patient flow through the Clinic. Works under the direct supervision of a physician/provider.

Job Specific Duties & Responsibilities:
  1. Performs nursing care activities according to established patient care protocols, standards and procedures.
  2. Admits patients for care with good history, medicine history and chief complaint.
  3. Provides data to assist in the development of nursing diagnoses which are central to the plan of care.
  4. Contributes to the development of approaches to meet the needs of patients and families, and, develops patient care plans utilizing a standardized nursing care plan and assists in setting priorities for care.
  5. Carries out planned approaches to patient care and performs common therapeutic nursing techniques.
  6. In collaboration with the registered nurse, assists with making adjustments in the care plan and reports outcomes of care to the registered nurse providing direct supervision.
  7. Participates in patient and family education as evidenced by teaching documentation with appropriate resources.
  8. Contributes to effective discharge and referral plans on admission utilizing appropriate hospital and community resources.
  9. Delegates patient care activities to appropriate personnel and maintains accountability for those activities.
  10. Performs all aspects of patient care in a manner that optimizes patient safety and reduces the likelihood of medical/health care errors; documents according to established standards; communicates effectively.
  11. Develops and maintains professional and clinical skills. Completes all annual competency and education requirements within established parameters and time frames. Participates in staff meetings, patient care meetings, educational in-services and other professional activities. Maintains awareness of policies and procedures and performance standards.
  12. Other duties as assigned.
Required Education and Experience:
  1. High school diploma or equivalent.
  2. Graduate of an accredited Licensed Practical Nurse program
Required Licenses, Certifications and/or Registrations:
  1. State of Washington Licensed Practical Nurse license, as required for the specific position assignment.
  2. Current Basic Life Support (BLS) certification, or within 180 days of hire.
  3. One year of experience working in an acute care facility.
