What will you do?

• The Registered Nurse will be responsible for providing aspects of Medication Management within the scope of his/her practice to clients that are receiving Outpatient Therapy/Counseling services and/or Rehabilitative Behavioral Health Services which include: (Behavioral Modification, Family Support, Behavioral Health Screening, Diagnostic Assessment Services, and Service Plan Development).

• All are medical or remedial services that have been recommended by a physician or other Licensed Practitioner of the Healing Arts (LPHA) within the scope of their practice under South Carolina State Law and as further determined by the SCDHHS for maximum reduction of physical or mental disability and restoration of a beneficiary to their best possible functional level. This role will report directly to the Medical Director and/or Clinical Director.

• Provides face-to-face and/or virtual contact with clients to monitor, evaluate, assess, and/or carry out the Agency Physician's orders regarding the psychological and/or physical problems and general wellness in accordance with the client's Individualized Plan of Care (IPOC).

• Conducts assessments to observe, monitor and care for the physical, nutritional, and psychological issues, problems or crises manifested in the course of a client's treatment; Provides assessment, testing, and referral for infectious diseases.

• Assesses and monitors the client's response to medication(s) to determine the need to continue medication and/or to determine the need to refer the client to a physician for a medication review.

• Assess and monitor a client's medical and other health issues that are either directly related to the mental health or substance related disorder, or to the treatment of the disorder (e.g. diabetes, cardiac and/or blood pressure issues, substance withdrawal symptoms, weight gain and fluid retention, seizures, etc.); may consult with client's caregiver in this regard, if necessary.

• Educate the client and/or family/responsible caregiver(s) on medications and potential medication side effects (especially those which may adversely affect health such as weight gain or loss, blood pressure changes, cardiac abnormalities, development of diabetes or seizures, etc.).

• Provides training to beneficiaries for self-administration of medication; and venipuncture required to monitor and assess mental health, substance disorders or related conditions; monitors side effects of psychotropic medications.

• Administers public relations and health education to promote awareness and understanding of the Agency's services; addresses community groups and other organizations interested in the Agency and/or mental health issues; attends conferences to maintain knowledge of professional trends and practices.

• Providing direct community based behavioral health services and supports, including Targeted Case Management, Rehabilitative Behavioral Health Services as well as therapy for the family as indicated in the Individual Plan of Care ?

• Assisting the team leader in monitoring and evaluating the effectiveness of interventions as evidenced by symptom reduction and progress toward therapeutic goals.

• Participating in the treatment planning process?

• Assist the team leader with consulting with identified non-medical providers, engaging community and natural supports and including their input in the treatment planning process.?

• Maintains strict confidentiality according to all Federal and State guidelines and requirements.?

• If a collaborative agreement is needed, CTS Health will provide an MD licensed in SC to fulfill this role.

Requirements

• Licensed by the South Carolina State Board of Nursing or medical examiners (as appropriate)

• Medical professionals must have minimum, educational requirements from a Board-approved education program and one year of experience working with the population to be served.

• Medical professionals providing services shall have documented evidence of CEUs annually, which are specifically related to behavioral health and medication management issues.

Communication abilities:

• Strong communication/verbal/written and listening skills.

Physical Requirements:

• Good physical strength

• Ability to stand for long periods of time.

• Ability to lift heavy objects.

• Use their hands to handle, control, or feel objects, tools, or controls.
