Position Summary:

The Clinical Program Specialist represents Roots Community Health Center, working as part of the Clinical Programs team, which implements programs for high priority conditions such as HIV, Hepatitis C, Opioid Use Disorder, and Diabetes. This position provides superior client/patient service, and interacts with team members/clinic personnel, employees of other departments, physicians’ offices and hospitals, as well as the public. The Clinical Program Specialist oversees a large client panel lists and uses prioritization skills to proactively track and coordinate patient visit, specialty referrals, medication pre-authorizations, and outreach activities in a timely and effective manner. This position will monitor the client/patient tracking systems and Electronic Health Records to make sure they are accurately maintained and to support reporting requirements and communication with the care team.

Duties and Responsibilities:
  • Outreach to hard-to-locate patients living in local encampments, shelters and temporary dwellings.
  • Collaborate with patients' navigation team members to coordinate care and share patient progress.
  • Maintain a patient tracking system on a weekly basis.
  • Coordinate Hepatitis medication treatment applications/preauthorization with the specialty pharmacy.
  • Work with the patient’s care providers to support the patient in recommended visit schedules for Hepatitis C, HIV, PrEP, Medication Assisted Treatment (MAT) for Opioid use disorder and other clinical program needs as they evolve.
  • Outreach to patients to educate them about PrEP (a medication to help prevent HIV), MAT, HCV treatment and other clinical programs under the direction of the Clinical Programs Coordinator.
  • Coordinate referrals for patients panel as needed.
  • Coordinate with Finance and Data staff to ensure accurate reporting.
  • Outreach and promote Roots events, campaigns and classes.
  • Provide relevant information to leadership to support the strategic development of the organization.
  • Attend regular program meetings to discuss status of service coordination, challenges/barriers to conducting the work, including hard to reach patients or workload.
  • Identify when an issue requires escalation to the next level of management and determine the best path forward.
Back up for Patient Care Coordinator
  • Verify insurance information/PCP assignment.
  • Verify and process referrals, authorizations, and links to appointments.
  • Participate in internal chart audits, billing audits, and other compliance programs.
  • Implement policies and procedures relating to patient services.
  • Implement new processes, projects, etc. as needed.
  • Attend and participate in MAA/TCM Implementation Trainings.
  • Attend organizational and other trainings and meeting related to job role.
Competencies
  • Minimum 1 years' experience performing clinical administrative functions.
  • Interest/experience working with low-income communities of color.
  • Excellent written and verbal communication skills.
  • Excellent team-player.
  • Ability to manage multiple phone lines, spreadsheets and files.
  • Ability to work with patients from different backgrounds.
  • Strong working knowledge of Microsoft Office and G-Suite (email, word processor, spreadsheets, forms).
  • Ability/willingness to learn Electronic Health Records and multiple reporting systems.
