Job Summary:
The Clinical Program Specialist plays a critical role in implementing programs for high-priority conditions at Roots Community Health Center. This position provides exceptional client/patient service, interacts with team members, and coordinates patient care to ensure timely and effective outcomes.
Key Responsibilities:
- Outreach to hard-to-reach patients in local encampments, shelters, and temporary dwellings.
- Collaborate with patients' navigation team members to coordinate care and share patient progress.
- Maintain a patient tracking system on a weekly basis.
- Coordinate Hepatitis medication treatment applications/preauthorization with the specialty pharmacy.
- Work with patients' care providers to support recommended visit schedules for Hepatitis C, HIV, PrEP, and other clinical program needs.
- Outreach to patients to educate them about PrEP, MAT, HCV treatment, and other clinical programs.
- Coordinate referrals for patients as needed.
- Work with Finance and Data staff to ensure accurate reporting.
- Outreach and promote Roots events, campaigns, and classes.
- Provide relevant information to leadership to support strategic development.
- Attend regular program meetings to discuss service coordination, challenges, and barriers.
Requirements:
- Minimum 1 year of experience in clinical administrative functions.
- Interest/experience working with low-income communities of color.
- Excellent written and verbal communication skills.
- Excellent team-player.
- Ability to manage multiple phone lines, spreadsheets, and files.
- Ability to work with patients from diverse backgrounds.
- Strong working knowledge of Microsoft Office and G-Suite.
- Ability/willingness to learn Electronic Health Records and multiple reporting systems.
