We are offering a special summer incentive for nurses with 1 or more years of experience, including a sign-on bonus of $5,000 and free health insurance for the remainder of the year. Vacation accrual is based on RN experience.

We are seeking a Chronic Disease Nurse Care Coordinator to promote effective partnerships between patients, families, nurses, physicians, and other healthcare providers to coordinate care for patients with chronic diseases. Responsibilities include managing care transitions, facilitating preventative care visits, providing clinical health coaching, and partnering with the care team for successful patient care.

Qualified candidates will demonstrate core values consistent with a patient-centered approach, effective communication skills, proactive patient advocacy, and a commitment to learning based on evidence-based practice guidelines. The role involves collaborating with practice leaders to implement effective internal tracking systems for patient care, ensuring all required elements are documented for billing, and participating in huddles with the care team to identify scheduling opportunities and care needs.

This position also entails providing clinical health coaching interventions to motivate patients and families toward successful self-management of chronic disease, developing care plans based on mutual goals with the patient and family, and facilitating patient access to appropriate medical providers. Attendance and participation in all Care Coordination related trainings and meetings is required.
