Works in collaboration with a physician and other palliative and hospice care team members in an active practice to deliver palliative and hospice care and related health care services to patients and families in a variety of settings.

Position Details:

  • Location: Hospital-based; Duval and surrounding counties
  • Hours: Monday - Friday; usually no weekends unless on-call
Job Responsibilities:
  • Provides medical expertise on pain, symptom management, and the evaluation of terminal prognosis to patient, family members and staff
  • Acts as the primary attending in collaboration with the palliative care/hospice physician for patients in hospice contract beds
  • Assist with the coordination of admissions to hospice contract beds by providing prognosis and review of diagnosis related to terminal illness
  • Assist with discharges as needed on selected hospice contract beds
  • Collects and assesses, patient care, data, develops and implements plan of care, manages, and evaluates outcomes of care.
  • Provide services related to Hospice and end of life care, assesses patient's needs and anticipates service requirements prior to discharge to GIP or home. Coordinates with hospice social workers, case managers and applicable ancillary staff regarding hospice care services.
  • Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation, and education for both patients and family.
  • Assists with diagnosis, treatment, and management of acute and chronic health conditions.
  • Orders, performs, and interprets laboratory and radiology tests within scope of professional practice.
  • Writes orders pertaining to informed consents, diagnostic procedures, and treatments. Prescribes medications and other treatments. Administers medications as prescribed.
  • Consults with palliative and/or hospice care physician or designee as needed, informs attending physician of services provided and collaborates with other physicians as needed.
  • Facilitates clarification of patient and family goals of care.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement of services provided.
  • Executes physician's orders in accordance with legal, ethical, and organizational standards.
  • Complies with applicable laws and regulations.
  • Initiates reimbursement for services rendered on a timely basis.
  • Ensure provision of informed consent for CPR and DNR.
  • Recommends medications as required to treat specific populations.
  • Adheres to the practice of confidentiality regarding patients, families, staff, and the organization.
  • Provides educational sessions to advance palliative and hospice care among internal staff, medical community, and community as a whole.
  • Participates in on-call coverage and assists in establishing schedule.
  • Participates in regular interdisciplinary team meetings to review active cases.
  • Adheres to internal controls established for the department.
  • Coordinates Inpatient APRN work assignments.
EDUCATION/EXPERIENCE:

Any combination of education and/or experience that would provide the required skill and knowledge for successful performance would be acceptable.

Typical qualifications would be equivalent to:
  • Graduate, advanced education with Master's degree.
  • Minimum of two (2) years nursing experience. Experience in hospice/palliative care preferred.
REQUIRED LICENSES/CERTIFICATIONS:
  • Current State of Florida driver's license with driving record acceptable to Community Hospice insurance carrier and reliable transportation.
  • Advanced Practice Nurse or PA licensed in the State of Florida
KNOWLEDGE/SKILLS/ABILITIES:
  • Patient care charts and patient histories
  • Health education theory and practice
  • Clinical operations and procedures
  • Current principles, methods, and procedures for the delivery of medical evaluation, diagnosis, and treatment in area of expertise
  • Excellent interpersonal and communications skills
