Job Summary The Registered Nurse, RN, is a member of the Interdisciplinary Team (IDT) and is pivotal in identifying the physical, psychological, social and spiritual needs of the patient and family. The RN initiates the appropriate interventions and support of the patient and family upon admission to Grace Healthcare Services and provides a continuous, appropriate, comprehensive and responsive plan of care. . Responsibilities and Duties Provide skilled nursing as determined by the plan of care. Coordinate the patient/family care and services including those of the IDT. Educate patient/family and other primary caregivers in appropriate care techniques. Serve as the patient/family advocate by maintaining regular communication with attending physicians, long term care facility staff, HMO case managers and others. Access physical and psychosocial status of the patient/family within 24 hours of admission to Grace Healthcare Services, update in Brightree. Develop and update plan of care with the IDT and others involved in the patients’ care. May participate in providing bereavement support to families and associated bereavement activities. Ensure and document appropriate use of medication, durable medical equipment and supplies. Participate in team meeting. Provide accurate documentation with visit itineraries to the office. Complete documentation as outlined in hospice, state and federal guidelines/standards. Provide appropriate coordinated care plans. Monitor the recertification/reauthorization process. Report necessary information to the Team Leader and other members of the IDT for on-call shift report. Respond to on-call issues as clinically appropriate. Perform on-site evaluations and supervision of Grace HHAs. Integrate quality improvement results into the established plan of care. Participate in quality assessment and improvement activities as assigned by the Clinical Director and/or Team Leader or QAP1 Manager. Participate in proactive patient calls and visits. All documentation is to be completed at point of service in Brightree and synced by end of business day. Participate in substantive chart reviews as assigned by the Team Leader. Participate in the orientation of new team members as assigned by the Clinical Director and/or Team Leader. Attend regularly scheduled in-service and educational conferences. Develop and achieve professional growth goals and objectives; review with supervisor on a regular basis. Represent professional nursing and Grace Healthcare Services in a positive manner. Participate in instructional activities with Grace Healthcare Services as well as outside the Grace Healthcare Services program. Promote a customer service oriented approach to care and customer relationships. Participate in customer conflict resolution. Qualifications and Skills NJ Registered Nurse License required Minimum two (2) years nursing experience in an acute-care hospital nursing in either medical, surgical or oncology unit or other commensurate experience Current clinical practice and a strong customer service focus Communication Skills Benefits and Perks The position includes health and dental benefits, paid time off, tuition reimbursement, and professional development.
