Key Responsibilities
- Effectively communicate significant clinical findings and changes in patient conditions to the interdisciplinary team.
- Conduct thorough assessments during face-to-face encounters, focusing on disease progression, functional decline, and patient and family coping mechanisms.
- Order and evaluate necessary diagnostic tests and procedures, documenting treatment plans and collaborating with physicians.
- Make appropriate referrals to other healthcare disciplines based on patient needs.
- Actively engage in interdisciplinary team meetings to enhance patient care.
Qualifications
- Must hold a current and unrestricted Nurse Practitioner license in the relevant state.
- A minimum of one year of experience as a Nurse Practitioner or hospice Registered Nurse within the last three years.
- Authority to practice and prescribe as a Nurse Practitioner in the state of service.
- Must have Medicare billing privileges as a Nurse Practitioner.
- Familiarity with Medicare reimbursement and coding regulations.
- Current CPR certification is required.
