Key Responsibilities
- Communicate important findings and changes in patient conditions to the clinical manager, attending physician, and interdisciplinary team members.
- Conduct face-to-face assessments, evaluating disease-related changes, functional decline, and quality of life for patients and families.
- Order and review necessary diagnostic tests and procedures, documenting treatment plans and collaborating with physicians.
- Make referrals to other healthcare disciplines as needed based on patient requirements.
- Actively participate in interdisciplinary team meetings.
Qualifications
- Current and unrestricted Nurse Practitioner license in the state of employment.
- At least one year of experience as a Nurse Practitioner or as a hospice Registered Nurse within the last three years.
- Authority to practice and prescribe as a Nurse Practitioner in the state of service.
- Medicare billing privileges as a Nurse Practitioner.
- Knowledge of Medicare reimbursement and coding for various service levels.
- Current CPR certification.
