Hamilton Arms Center in Lancaster, PA is currently growing their MDS team and is looking for a Full Time Licensed Practical Nurse Assessment Coordinator (LPNAC) to assist with leading our MDS team The position is on site at the facility, and remote work will not be considered due to the collaborative nature of the role with the MDS team on site. The LPNAC must be confident in assessments and MDS skills. The LPNAC must be familiar with PPD (PDPM) and medicare/medicaid billing and reimbursements. Unqualified candidates will not be considered.

Requirements of the Licensed Practical Nurse Assessment Coordinator (LPNAC):
  • Must be confident in MDS
  • Must have previous experience as a LPNAC and assessments
  • Licensed Practical Nurse (LPN) with effective leadership & assessment skills in a long-term care setting
  • Must have knowledge of state and federal regulations
  • Proficient in PA CMI
  • Licensed Practical Nurse Assessment Coordinator (LPNAC) must have Medicare billing knowledge and experience.
  • Knowledge of PPD (PDPM)
  • Experienced in Insurance updates
  • Must successfully complete a post offer, pre-employment criminal background check and physical / drug screen.
Responsibilities of the Licensed Practical Nurse Assessment Coordinator (LPNAC):
  • Conduct and coordinate the completion of the resident assessment in accordance with the requirements of this state and policies and goals of the Facility.
  • Conduct and coordinate the completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs.
  • Develop, implement, and maintain an ongoing quality assurance program for the resident assessment.
  • Participate in Facility surveys (inspections) made by authorized government agencies.
  • Monitor the Facility's QI and QM reports to ensure that appropriate corrective action can be implemented when potential problem areas occur.
  • Provide initial and ongoing education with the interdisciplinary team involved in the RAI process.
