Job Description

Assigns all principal. secondary diagnoses and procedural ILCD-9-CM codes utilizing the 3M HDM coding system to all Emergency Room encounters for hospital reimbursement.

Assigns procedural codes on Emergency Room case according to CPT4 coding guidelines

Assist with the Emergency Room Coding.

Abstracts and verify patient's demographics.

Works with and queries the physician in regards to documentation issues that impacts coding.

Keeps abreast of all current coding changes and other related topics.

Utilizes the EMR system to facilitate coding.

Utilizes the electronic chart tracking system.

Work with the Ambulatory Surgery staff to ensure all ambulatory surgery cases are captured coded and closed for billing.

Other and all duties as assigned by the supervisor.

Review the patient's medical record (inpatient, ambulatory surgery and Emergency Room visits) to determine appropriate codes. Uses the HHC approved coding system to enter ICD-9 and CPT-4 codes.

Duties are predominately sedentary in nature. Incumbent must use several different computer systems.

Minimum Qualifications
1. Bachelor's Degree from an accredited college or university in a Health Information Administration (HIA) or Health Information Technology (HIT) program accredited by the American Health Information Management Association (AHIMA) and six (6) months experience in coding and abstracting medical records in a recognized hospital or health care organization; or
2. High School diploma or its educational equivalent; and
3. Completion of a coding, medical record technology or health information technology program approved by the American Health Information Management Association (AHIMA) and one (1) year of experience in coding and abstracting medical records in
a recognized hospital or health care organization, six (6) months of which must have been in a supervisory capacity; or
4. Possession of a valid certificate as a Certified Coding Specialist (CCS) from the American Health Information Management Association (AHIMA) and one (1) year of experience in coding and abstracting medical records in a recognized hospital or health care organization, six (6) months of which must have been in a supervisory capacity; or
5. Three (3) years of satisfactory experience in coding and abstracting medical records in a recognized hospital or health care organization, one (1) year of which must have been in a supervisory capacity.
6. A satisfactory equivalent. However, all candidates must be high school graduates or possess a high school equivalency diploma.
