Clinical Assessment Provider - SMI/SED

Job Purpose: The Clinical Assessment Provider meets with new, returning, or prospective patients, asks a series of clinical assessment questions for the purpose of compiling an integrated whole-person behavioral health assessment, then uses this data to determine the best next course of action for that patient.

This is an exempt position that is part of the Behavioral Health Team and reports to their assigned Clinical Director.

What You Will Do:
  • Completes SMI/SED evaluations within 7 days of patient's request
  • Submit necessary SMI/SED documentation to Solari, staff/coordinate as necessary to support determination process and inform clinical team and client of the outcome
  • Additional administrative steps to coordinate/communicate with patients identified as SMI/SED candidates by their insurance plan
  • Interviews patients, in a courteous and professional manner, to obtain pertinent clinical information.
  • Uses the DSM-5 to compile and identify patients' clinical diagnoses.
  • Completes screening of patient's needs and symptoms to determine the appropriateness of outpatient behavioral health services
  • Assesses for risk for all patients and provides safety/crisis planning and resources to patients who identify danger to self or others.
  • Processes and completes documentation for patients efficiently and in accordance with protocols.
  • Outreaches patients who do not attend scheduled appointments and attempt to reschedule within 24 hours.
  • Answers and screens inquiry calls and emails from prospective patients.
  • Ensures that all admission forms are properly signed and filed and that HIPAA procedures are followed.
  • Communicates and coordinates with other team members constructively in order to reduce conflict and enhance the resolution of issues.
  • Respect patient dignity and confidentiality.

What We Need From You:
  • Bachelor's or master's degree required in a related field. (Bachelor's in nursing with at least 1 year of behavioral health experience or a psychiatric-mental health certification or a Master's degree in Counseling, Social Work or Marriage and Family Therapy)
  • Clinical Assessment Provider
  • Experience using the DSM-5 for behavioral health diagnoses.
  • Ability to unify and motivate teams to accomplish goals and objectives.
  • Counseling training and experience a plus, licensed by the Arizona Board of Behavioral Health Examiners or
  • Nursing board required.
  • Experience with assessment of safety and crisis processes in the state of Arizona preferred.
  • Telehealth experience a plus.
  • Knowledgeable of behavioral health services and the general mental health population.
  • Exceptional communication skills and ability to de-escalate with the use of discretion required.
  • Accurate data entry skills into an electronic medical record and an ability to multi-task multiple priorities simultaneously.
  • Bilingual applicants encouraged to apply.
  • Independent License Therapist strongly preferred.
