The Outreach Coordinator is responsible for all marketing and sales activities relating to developing, building, and maintaining existing professional relationships within communities we serve as well as cultivating new relationships aimed at building/expanding Long Term Respite Grant activities. The individual is also responsible for developing and maintaining contracts with community programs & facilities and all other available services provided by the Grant. This position will conduct telephonic and field outreach consisting of but not limited to establishing relationships with Nursing Homes, Adult Living Facilities, Social Day Programs for respite services, attending community events, engaging with new and existing referral sources and coordination of services. The Outreach Coordinator represents the entire Nascentia Health system and must be familiar with, and able to effectively explain, communicate, and facilitate referrals to assist all Nascentia Health services.

Performance Responsibilities and Standards:

  1. Develop and maintain professional relationships with clients, providers and community partners.
  2. Cultivate new professional relationships with facilities and programs to provide options to clients.
  3. Represent Nascentia Health and all its entities at community outreach events/sponsorships.
  4. Assist with development and coordination of presentations.
  5. Meet with eligible participants with the goal of education, presenting product information, and assisting with coordination of services.
  6. Provides ongoing assistance to clients as necessary, answering questions and/or directing inquiries to Nascentia Home Aides staff.
  7. Screen client eligibility, review and validate documentation necessary for participation in Long Term Respite services.
  8. Assess the needs of the community for caregiver services and provide information to management.
  9. Ensure reporting and attend meetings regularly to ensure current information sharing.
  10. Serve on the Long Term Respite Committee and take lead on needed initiatives that fall within scope of practice.
  11. Promote a positive customer service approach to client and employee contact, internally and externally.
  12. Other duties as assigned.
Requirements:
  1. Associates Degree required in health-related or human services; or Bachelor's Degree preferred.
  2. Customer/client Focus. Strong interpersonal skills in working with the public and seniors.
  3. Must be able to work independently, to plan and coordinate services..
  4. Excellent writing skills with strong interpersonal communication and problem-solving skills.
  5. Strong presentation skills.
  6. Ability to support and interface with a wide cross section of internal and external audiences with varying levels of experience.
  7. Strong working knowledge of, and experience using a wide variety of software platforms including Microsoft Office Suite (Outlook, Word, Excel, and PowerPoint).
  8. Self-starter who is highly organized and motivated with the ability to multi-task.
  9. Ability to drive and travel (must have valid NYS driver's license).
  10. Must ensure compliance with all regulatory, audit and corporate policies.
  11. Professional appearance and dress.
Physical Requirements:
  1. Must be able to drive and travel on a regular basis.
  2. Speaking/visual/hearing ability must be sufficient to comprehend and express written/verbal communication.
  3. Frequent sitting, standing, walking.
  4. Extended periods of computer use.
  5. Ability to express self verbally and in writing.
  6. Ability to verbally communicate over a telephone.
  7. Bloodborne Pathogens Exposure Category III - Employee who rarely has contact with blood and body fluids.
