Job Summary

Menorah Park, a leading healthcare provider in Syracuse, New York, seeks an experienced Licensed Practical Nurse (LPN) to fill the role of Full Time LPN Care Coordinator. This position plays a vital part in coordinating and managing the care of our residents, ensuring they receive the highest quality of care.

The successful candidate will work closely with our healthcare team to develop personalized care plans, guaranteeing that all medical needs are met. This is a fast-paced position requiring excellent communication and organizational skills, as well as the ability to work independently and efficiently in a dynamic environment.

Responsibilities
  • Plan safe, economical, efficient, and therapeutic care for residents.
  • Coordinate care received by clients, ensuring seamless delivery of services.
  • Develop and implement educational programs for clients and their families, reinforcing teaching and supervision of procedures as necessary.
  • Provide direct client care, adhering to established protocols and guidelines.
  • Implement all phases of the nursing process, including assessment, planning, implementation, and evaluation.
  • Document all information related to client care, maintaining accurate and comprehensive records.
  • Recognize changes in client status and report to appropriate personnel, ensuring timely interventions.
  • Act as liaison between clients, families, and physicians, facilitating effective communication and coordination.
  • Maintain communication and coordination among all services provided, ensuring seamless delivery of care.
  • Assist in planning for discharge of clients when necessary, ensuring a smooth transition to next level of care.
Requirements
  • Active LPN license in the state of New York.
  • Minimum of 1 year of experience in a long-term care or healthcare setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently and efficiently in a fast-paced environment, prioritizing tasks and managing time effectively.
  • Proficient in electronic medical records systems, with knowledge of relevant software and hardware.
  • Excellent organizational and time-management skills, with attention to detail and ability to multitask.
  • Demonstrated empathy, compassion, and patience towards residents and their families.
Compensation & Benefits

This is a full-time, hourly position with a salary range of $25-$30 per hour, paid on a weekly basis. Employees will also receive a comprehensive benefits package, including medical, dental, and vision insurance, as well as paid time off and a retirement plan.
