We are seeking a highly motivated and experienced Case Manager in Riverside County to join our team. The Case Manager will be responsible for supporting caregivers enrolled in community college. They will work closely with other staff members, volunteers, and community partners to ensure the successful delivery of our support of UDW's partnership with the California Community Colleges Chancellor's Office.

The Case Manager does not need to live in Riverside, but must live in Riverside County.

Job Duties and Responsibilities:

  • Meet individually with students to assess needs and connect them to appropriate services and resources
  • Maintain updated records and case notes on all students accessing services
  • Research and build partnerships with community organizations to provide wrap-around services such as healthcare, counseling, housing, financial assistance, academic support, etc.
  • Develop and facilitate regular peer support groups focused on topics like stress management, time management, confidence building, etc.
  • Create and present workshops focused on soft skills like communication, collaboration, critical thinking, etc.
  • Collect data and track student usage of services to identify trends and areas of improvement
  • Attend community resource fairs and events to expand network of services
  • Maintain client confidentiality and ethical standards
  • Hybrid schedule, must work a minimum of 2 days per week in the office

Work Qualifications:

  • Bachelor's degree in social work, counseling, or related field
  • 2+ years of relevant experience in case management, counseling, or social services
  • Knowledge of community resources and services
  • Strong presentation, communication and interpersonal skills
  • Ability to collect data, write reports, and identify trends
  • Passion for empowering students and connecting them with resources

Required Bilingual Language(s) in one or more of the following: 

• Spanish  

• Vietnamese  

• Arabic
