What will I be doing?

  • As an Intake Coordinator, you will be responsible for completing assessments and necessary documentation.
  • Receive and process incoming referrals for mental health services.
  • Complete CFARS/FARS.
  • Develop individualized treatment plans that address diagnosis(es) and other presenting problems.
  • Conduct intake assessments to document history, diagnose, and identify issues for treatment by interviewing the child, family and other interested parties including, but not limited to educational staff, DJJ representatives and case managers.
  • Work collaboratively with the assigned DPO and clinical staff to support business and administrative needs.
  • Conduct telephone intake with follow-up assessment interviews on appropriate referrals and ensure the receipt of a referral information.
  • Maintains ongoing relationships with funding agencies and develops new resources through marketing presentations ensuring sufficient amount of referrals for program stability and growth
  • Coordinates the administrative functions for the program such as answering calls, greeting visitors, entering required data into EMR system.
  • Position will be in Office / Telemedicine.
  • This is a Fee for Service position that pays $95.50 per intake completed.

  • Must possess general computer skills in Microsoft Word, Excel, Power Point.

Qualifications:

  • Master's degree in Human Services discipline, including, but not limited to: Social Work, Psychology, Sociology, Counseling required
  • Registered Intern or Eligible to become one in the state of Florida (preferred)
  • 2 years’ experience working with children (experience can include internships, volunteer work, etc.) (preferred)
  • Must be Bilingual in English and Spanish language - required
