Description

The Italian Home for Children is a dynamic social service organization committed to improving the well-being of our community. We believe in a holistic approach to health, focusing on education, prevention, and community empowerment. As part of our mission, we are seeking a passionate and dedicated Community Health Worker to join our team.

As a Community Health Worker (CHW), you will play a vital role in promoting health and wellness within our community via school and local housing authority partnership. You will serve as a liaison between individuals and community resources, providing support, education, and advocacy to improve health outcomes. Your responsibilities will include but are not limited to:

ESSENTIAL FUNCTIONS & RESPONSIBILITIES:

  • Conducting outreach efforts to identify community health needs and establish relationships with community members.
  • Providing health education and resources to individuals and groups on topics such: navigating life stressors and coping skill strategies.
  • Assisting youth community members in navigating the healthcare system, including scheduling appointments, understanding insurance coverage, and accessing financial assistance programs regardless of insurance.
  • Conducting home visits and assessing individuals' needs and provide personalized support on challenges to school attendance and school performance.
  • Collaborating with healthcare providers, social service agencies, and community organizations to coordinate care and address social determinants of health.
  • Advocating for policy changes and community initiatives that promote health equity and address underlying health disparities.
  • Maintaining accurate records of client interactions and program activities and participating in data collection and reporting efforts.
  • Attend Community Events to Promote and Represent IHC
  • Tracking, Evaluating, and reporting data for program.
  • In-person in school 3 times weekly.
Qualifications
  • Must be 21 years of age or older
  • Bachelor's degree in a human service field from an accredited university and 1 year of related experience working with youth OR
  • Associate degree in a human service field from an accredited school and 1 year of related experience with target population
  • Bilingual language capacity preferred
  • Passion for supporting youth struggling with social, emotional, and behavioral struggles.
  • Demonstrated ability to model, teach, and support, with excellent interpersonal skills
  • Superior organizational skills, strong work ethic, and attention to detail
  • Demonstrated ability to accept and provide constructive feedback
  • Sensitivity and respect for diversity in both clients and staff
  • Unique ability to connect with others, with clear healthy boundaries
  • Sound and appropriate judgment
  • Solid writing and documentation skills, as well as computer literacy
  • Maintain strictest confidentiality regarding clients, families, and employees; adhere to all HIPPA guidelines and regulations.
  • Visual and hearing acuity to perform job related functions.
  • Strong commitment to Italian Home's values, philosophy, and mission
  • Previous experience in community outreach, health education, or social services preferred.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport with diverse populations.
  • Knowledge of local resources and social services available to community members.
  • Ability to work independently and as part of a team, with excellent organizational and time management skills
  • Commitment to cultural competency, confidentiality, and ethical standards in healthcare delivery.
