Job Title: Physical Therapist
- Full-time, part-time, and PRN availability.
- Monday - Friday flexible hours.
- Rotating call every 6th Saturday.

About the Role:
As a Physical Therapist at Columbine Poudre Home Care you will:
- Provide appropriate therapy services per physician's orders and communicate treatment plans with clients, families, and medical staff.
- Participate in meetings, discharge planning, and quality assurance programs, while ensuring proper documentation and compliance.
- Provide in-services, refer clients to outpatient care, and perform duties for other agencies under supervision, utilizing their knowledge of physical therapy techniques and equipment.
What We're Looking For:
Experience:
- Graduate of accredited school of Physical Therapy, bachelor's or greater degree required.
- Licensed by the State of Colorado as a Physical Therapist.
- 1+ year of experience as a Physical Therapist.
Skills:
- Ability to evaluate physical conditions, understand patient limitations, and develop customized treatment plans.
- Expertise in physical therapy techniques such as stretching, exercise programs, and manual therapy.
- Proficient in explaining treatment plans and progress to patients, families, and healthcare teams in a clear and empathetic manner.
- Accuracy in maintaining patient records, progress notes, and treatment evaluations in compliance with healthcare regulations.
- Familiarity with physical therapy tools, modalities, and software used in treatment and patient tracking.

Our Total Rewards package includes a salary range of between $33 - $38/hr., with the opportunity to earn up to $150 or more for admitting new clients
