About the Role:

As a CNA at Columbine Poudre Home Care, you will:
  • Deliver personalized care to clients in their homes, ensuring their physical, mental, and psychosocial well-being as per individualized care plans.
  • Assist with daily activities such as grooming, hygiene, feeding, toileting, and mobility, while maintaining clients' dignity and comfort.
  • Support the ability of home health care clients to continue to live safely in their own homes.
  • Observe and report changes in residents' conditions or behaviors to nursing staff.
  • Maintain residents' skin integrity through proper care techniques.
  • Accurately document care provided and adhere to safety and infection control procedures.
  • Provide restorative care to help residents maintain or regain independence.
  • Participate in care planning meetings.
  • Perform other duties as assigned.
What We're Looking For:

Experience and Education:
  • Minimum 18 years of age.
  • High school diploma or GED preferred.
  • A Certified Nursing Assistant License in the state of Colorado.
  • 3 months of CNA experience required.
Skills:
  • Ability to effectively able to read and interpret documents.
  • Ability to communicate effectively.
  • Basic mathematical skills.
  • Ability to reason and problem solve.
  • Basic technical skills.
  • Knowledge of the aging process, dementia, and disabilities.

Our Total Rewards package includes a salary range of between $19 - $22/hr. depending on experience. We also offer a travel stipend and evening shift differential
