Key Responsibilities:
- Comprehensive Patient Assessments: Evaluate new patients within 8 hours of admission to ensure timely care.
- Personalized Nursing Care: Deliver individual nursing care as prescribed by the physician.
- Medication Management: Oversee daily medication administration, including monitoring and storage.
- Collaborative Treatment Planning: Participate in the creation of individualized treatment plans with an interdisciplinary team.
- Detailed Documentation: Maintain accurate progress notes for both one-on-one and group sessions in line with policies and procedures.
- Risk Management: Help coordinate activities aimed at reducing or eliminating risks related to patient, staff, and visitor safety.
- Admission Processes: Complete and sign necessary admission paperwork and other documentation as required by law and hospital policies.

Qualifications:
- Licensing: Valid Registered Nurse (RN) license in Louisiana; CPR, CPI, and BLS certifications required.

- Clinical Skills: Strong clinical skills and knowledge of home health care practices.
- Communication: Excellent communication and interpersonal skills.
- Tech-Savvy: Knowledgeable in using EMR systems.
- Independence: Ability to work independently and manage a flexible schedule.
- Transportation: Reliable transportation and a valid driver's license.
