Responsibilities:

  • Provide comprehensive case management services to clients and families
  • Develop and negotiate case plans in collaboration with parents and relevant parties
  • Conduct regular home visits and document all case activities accurately
  • Submit service requests and make timely referrals to services
  • Attend court activities as required and complete court documentation within specified timeframes
  • Conduct parental and sibling visits when necessary
  • Transport children as needed and ensure completion of Independent Living functions

Qualifications:

  • Bachelor's degree in Human Services or related field
  • Minimum one year of relevant experience
  • Excellent written and verbal communication skills
  • Proficiency in Microsoft Office programs
