As a Home Health Registered Nurse RN Case Manager, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing compassionate and personalized care to patients and their families, and we are seeking a skilled and compassionate RN to join our team.

About the Role

This is a full-time position, 40 hours per week, with a competitive salary and benefits package. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient. Your expertise and experience will be valued and respected, and you will have the opportunity to make a real difference in the lives of our patients and their families.

Responsibilities

  • Coordinate and direct the care of a caseload of home patients, adapting to the changing needs of each patient and family.
  • Assess the physical, functional, psychosocial, social, spiritual, educational, developmental, cultural, and cognitive status of each patient, and develop individualized care plans that meet their unique needs.
  • Provide comprehensive assessment, planning, implementation, and evaluation of care for each patient, working closely with our interdisciplinary team to ensure seamless care coordination.
  • Collaborate with patients, families, caregivers, and community providers to educate and support them in providing safe and effective care.
  • Identify performance improvement and home health standard of care initiatives, and assist in designing or implementing programs to address needed changes.

Requirements

  • Current Registered Nursing License in State of Practice
  • Minimum of one-year relevant clinical RN experience
  • Strong computer and technology skills
  • Valid Driver's License and current car insurance
  • CPR certified

Preferred Qualifications

  • A working knowledge of community resources and an ability to refer patients and families appropriately
  • Home Care Regulations and Third-Party Reimbursement as it impacts care delivery
  • Current IV Therapy skills
  • Home Health Case-Manager Certification
  • COS-C
  • Recent, relevant experience in a Medicare-certified home health agency as a case-manager
  • Bachelor's degree in nursing
