Position Title: Medical Coordinator

Reporting Relationship: Lead Clinician

Position Summary

The Medical Coordinator provides children with a positive medical environment. The Medical Coordinator ensures proper delivery of medical, dental, mental health, and pharmacy services on behalf of children in care. The Medical Coordinator regularly communicates with the cross-functional team and strives to meet continuous quality improvement measures.

RESPONSIBILITIES

  • Provide children with a positive and appropriate educational environment consistent with the National Youth Advocate Program’s mission, values, and philosophies.

  • Ensure confidentiality of health records and adhere to NYAP, State, and Funder guidelines for protection of protected health information (PHI)

  • Work to obtain Memorandum of Understandings (MOUs) with medical, dental and mental health providers.

  • Participate in intakes and discharges of children, including administering Covid 19 testing, coordinating physical assessments, preparing intake and discharge medical packages, scanning/uploading documents in portals and health record systems.

  • Continuously asses the children’s’ medical, dental, mental health in accordance with NYAP and other policies and procedures.

  • Collaborate with children’s’ assigned cross functional team and medical, mental health, dental, and pharmacy providers to provide appropriate care to the children including but not limited to: assisting health providers in completing PCU process to become an approved provider; making initial, follow-up, and wellness appointments; preparing Treatment Authorization Requests (TAR) prior to appointments and follow up to ensure payment; transporting children to and from medical appointments and providing supervision of the children during appointments; obtaining medical clearances.

  • Report medical issues via SIR process and elevate issues of medical concern to the supervisor, assigned cross functional team as appropriate and the Division of Health for Unaccompanied Children (DHUC) for consultation and guidance.

  • Support foster parents in education related to medical policies, answering questions related to medical care of children and assisting them with required medical responsibilities.

  • Collect health records from medical providers and ensure they are thoroughly completed, scanned/uploaded into the portals and health record systems; print Authorizations for medical, mental health and dental treatment and providing them to foster parents.

  • Complete and submit all assigned documentation in the expected timeframe including such things as placement documents, assessments medical packages, COVID 19 checklist, temperature logs, medical summary form, medical checklist for Transfers, etc. Uploaded documents to portals and health record systems and ensure physical health files and records are properly filed in secured location complying with confidentiality of health records.

  • Regularly communicate with the cross-functional team and strive to meet continuous quality improvement measures related to medical records, and timeliness of uploads.

  • Responsible for taking temperatures of children, staff, and visitors, provide Covid 19 screening questionnaire, and follow all CDC, Federal, State, and Field Guidance policies related to medical and Covid 19 standard precautions.

  • Maintain medicine cabinets and first aid kits with OTC medication, first aid inventory, rapid antigen Covid 19 Test inventory, and other medical items.

  • Provide in-service training and continued education to staff and foster parents on health- related needs of adolescents, five rights of medication administration, Covid-19 testing, CPR, and First Aid. Maintain Red Cross CPR/First Aid Trainer’s Certification.

  • Other duties as requested

MINIMUM QUALIFICATIONS

  • Highschool Diploma or bachelor’s degree.

  • (1) year experience working in a medical, dental, mental health or physician’s office.

  • Experience completing medical appointments and medical documentation.

  • Experience working with children and adolescents is preferred.

  • Bilingual in English/Spanish fluency is required.

  • Recognizes and understands immigrant population and provides emotional and physical comfort and safety to minors.

  • Demonstrated ability to maintain liaisons with community health providers and expand community health networks.

  • Must be open and willing to work non-traditional work hours and be available for on-call phone consultations as needed.

  • Must have a valid driver’s license, excellent driving record, a safe and clean personal vehicle, and current automobile insurance.

  • Must be able to maintain compliance with attending agency required training programs.

  • Bilingual English/Spanish fluency preferred.

  • Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.

  • Reliable transportation with a safe driving record.

  • Reliable, working telephone.

  • Experience working with diverse groups.
