Job Summary

Coordinate with the resident and resident representative to review policies on Advanced Directives.

Completes MDS (Minimum Data Set)

Arranges for appropriate services to meet the needs of the residents i.e. psych, dental, vision, etc.

Begins discharge planning on day one. Assists residents with finding appropriate housing, arranging for appropriate equipment for discharge back into the community.

Assists residents and families cope with the effects of the resident living a long-term care facility and temporary restricted visitation.

Perform audits in response to reportable events and performance improvement plans.

Ensure excellent communication among residents, families, and staff.

Create positive resident and staff outcomes.

Respect others by treating them the way we want to be treated.

Demonstrate kindness, compassion, honesty, and integrity at all times

Assesses residents, including mood, behavior and psychosocial issues, family relationships, coping mechanism, reactions to institutional placement, and assessment of level of care requirements.

Works in collaboration with physicians and other health care personnel in resident evaluation and treatment.

Evaluate social and family information and assist in determining plans for social treatment.

Refer residents/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.

Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures.

Documents on charts, resident care forms, etc. on a timely and concise basis and ensure that all documentation in informative and descriptive.

Completes the social history/assessment on each resident upon admission and follows through with an annual update.

Submits appropriate Notices of Non-Coverage (ABNs) and Notice of Medicare Non-Coverage (NOMNC) to residents or their responsible parties.

Works with hospital, families and other agencies to receive resident referrals.

Required skills and qualifications:

Bachelor of Arts in Social Services or related Field.

Minimum of two (2) years' experience in a long-term care setting.

Licensed Social Worker (LSW) preferred.

Must have outstanding critical thinking skills and outstanding customer service skills.
