Wellness Nurse

- Performing nursing procedures within the scope of practice as allowed by state law and in compliance with Villas of Holly Brook.
- Ensure sound infection control practices are in place that promotes a healthy environment.
- Contribute to resident assessments and customize service plan development.
- Lead the medication administration process by correctly transcribing physician orders, correctly ordering medication and completing error free medication reminders.
- Provide first aid and emergency assistance while on duty within the community.
- Participate in resident and family care conferences.
- Maintain all privacy rules and regulations pertaining to the Health Insurance Portability and Accountability Act (HIPPA)
- Ensure that changes in resident condition are evaluated, monitored, documented and that effective interventions are implemented.
- Review resident records on a regular basis and ensure that documentation accurately addresses the healthcare issues, including identified concerns, interventions and outcomes, of all residents.
- Ensure that resident care is provided in a safe and effective manner, while consistently maintaining resident dignity, choice, privacy, and respect.
- Maintain a current knowledge of OSHA and state regulations and routinely monitor Community compliance with regulations-related to resident care and nursing practices.
- Any other duties assigned by Wellness Director.
- Assist with supervising Wellness Department employees in providing day-to-day care.
- Provide orientation and ongoing training as requested.
- Communicate and enforce policies and procedures including Villas of Holly Brook policies and procedures dealing with rules of employee conduct, the employer's safety and sanitation policies and other policies.
- Scheduling podiatry visits, assist podiatry during the visit, if necessary, complete documentation in PCC
- Incident reports: Complete following an incident, notify PCP, provide documentation and timeline to Wellness Director following company policy for incident follow-up

- Medications: check in medications, add or remove any medications based on changes, fax new orders to pharmacy, deliver medications as needed to resident apartments and place in locked cabinet, pre-fill insulin syringes, remove medications after resident moves out, weekly narcotics count, administer medications if resident is unable to do it themselves
- TB testing: administer 2-step TB for any employee or resident who needs it
- Obtain physician certifications
- Other duties as assigned by Wellness Director
- Perform job duties for residents and with team members in a courteous and professional manner.
- Take initiative to ensure resident safety and satisfaction is a priority.
- Meet and greet visitors in a friendly, helpful manner (Visitors include anyone who visits the community; specifically, current residents' families and friends, prospective residents and their families referral sources, vendors and regulators).
- Answer phones appropriately, according to company and Community standards.
- Communicate professionally and cooperate with Executive Director and all Community personnel.
- Ensure compliance and understanding of all regulations regarding residents' rights.
- Attend all required in-service and training
- Follow & communicate Villas of Holly Brook policies and procedures.
- The job description provides a framework for the job; other duties may be assigned as necessary.
- Licensed RN/LPN in good standing with State Licensing Authority.
- Experience of one year or more in the care of senior adults and adults with cognitive decline is preferred.
- Ability to communicate effectively with residents, families, staff, community and State officials and general public.
- Must have compassion for and desire to work with the elderly.
- Must demonstrate the ability to work responsibly as a team member as well as an individual.
- Must be honest, fair, dependable, respect confidentiality and the rights and privacy of others.
- Must practice and promote Villas of Holly Brook Policy and Procedures and Mission Statement.
- Must have exceptional organizational skills and ability to re-prioritize daily tasks in order to accommodate fluctuating needs of the resident and Community.
- Must have strong documentation skills.
- Must pass criminal background check.
- Must meet all health requirements.
- Must be able to perform duties and responsibilities (essential job functions), with or without reasonable accommodations.
- Medium work; ability to exe1i 20-50 pounds of force occasionally, and /or 10-25 pounds of force frequently, and /or greater than negligible up to 10 pounds of force constantly to lift, carry, push, pull or otherwise move objects.
- Must be able to relate to and work with the ill, disabled, elderly, emotionally upset and at times hostile people within the community.
