Primary responsibility of the Service Support Coordinator is to provide services to individuals & families who are affected by a substance use disorder. This position will combine and utilize skills in the area of non-clinical coordination of outpatient care. The position will provide support to providers and staff to be able to serve their patients in an efficient manner.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. High School Diploma or equivalent

EXPERIENCE:

1. Two (2) years' experience working in customer service, a health care setting and/or record keeping.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Associates degree or higher in the human services field or health care.

EXPERIENCE:

1. Experience with electronic medical records, insurance authorizations.

1. Assist in obtaining prior authorizations from third party carriers for treatment & refill requests.

2. Maintaining provider scheduling & performing all registration duties to keep electronic medical record current.

3. Maintain all tracking spreadsheets utilized for patient treatment team meetings & compliance.

4. Performs various administrative duties, including answering the telephone, compiling data, preparing reports and correspondence, copying and filing documents, procuring supplies, attending meetings.

5. Assisting in the preparation of discharge letters.

6. Serve as a liaison to community agencies to & resources by facilitating regular contact & collaboration with these partners on a monthly basis.

7. Ensure insurance & OMBAT regulations & codes are accurate by familiarizing worker of insurance regulation changes & yearly OBMAT regulations & certification.

8. Participate & complete NetLearning modules to assist in knowledge of hospital, medical center and/or health system resources to access and provide for patient care needs.

9. Participate in multi-disciplinary health care teams to update team on outstanding pre-authorizations, provider treatment plans & patient compliance.

10. Ensures complete, accurate documentation of insurance review & patient contacts as required.

11. Meets with clients and consults with service providers on a regular basis to ensure the proper implementation of case management plans and to monitor patient progress.

12. Provides patients to and/or coordinates with other private or public mental health and medical service providers and support programs as appropriate (i.e DHHR for insurance review).

13. Provides information and support regarding medication compliance, urine drug screens and medication counts.

2. Able to bend, stoop, reach and be capable of normal rotation, standing and walking for an 8 - 12 hour shift

SKILLS AND ABILITIES:

1. Strong interpersonal communication skills and an ability to provide services to individuals affected by substance use.

2. Ability to work within a multidisciplinary team.

3. Good written & oral communication skills.

4. Ability to use basic computer programs such as MS Excel, Word

5. Excellent organizational skills.

6. Ability to comprehend and perform oral and written instructions and procedures.

7. Effective reading and comprehension skills.

8. Must have strong communication skills, written and verbal.

9. Ability to work successfully under stressful conditions and must be capable of adapting to varying workloads and work assignments on a constant basis.
