Responsibilities The Inpatient Social Worker carries a patient caseload at the 101 bed Psychiatric Hospital in Brooklyn Park, Minnesota, serving ages primarily 5-26. This role is part of a four year SAMHSA grant-funded position to primarily serve Native American patients. This role focuses on facilitating groups, treatment planning, discharge planning, care coordination, and communication with caretakers, community providers, and others involved in the care of patients. This role will specialize in easing Indigenous Health Disparities within the treatment of mental health and hospitalization, located within the hospital (75% of the time) and in the community (25% of the time). The role also includes community outreach to the Native American community and providing social work services across the continuum of care, including their return to the community. Responsibilities and Accountabilities: Accountable for upholding and applying clinical knowledge and understanding of emotional and behavioral health practices and principles, including Indigenous Health Disparities Assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the treatment team throughout the continuum of care, including 3-6 months post-hospital discharge within the community setting Develop and revise treatment and complex discharge plans and communicate these with patients, caretakers, tribal entities, the treatment team, and direct care staff Provide psychoeducation to patients, caretakers, the treatment team, and direct care staff regarding diagnosis, symptom management, recommendations, discharge plans (including leading discharge meetings), safety plans, patient rights, financial impact issues, Indigenous Health Disparities and other pertinent patient care matters across the continuum of care, including in the community Communicate effectively alongside the Spiritual Cultural Support Liaison with external providers, agencies, tribal entities, and government and private payer networks to develop relationships and refer patients to the next appropriate levels of care following hospitalization Responsible for leading child, adolescent, and young adult therapy/process groups on the inpatient units Within the training process, complete Think: Kids: Collaborative Problem Solving (CPS) training and recognize that Kids Do Well If They Can. Social Workers participate in using the CPS Assessment and Planning tool to help kids develop skills for their challenging behaviors. Oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care Provides culturally specific and trauma-informed social work and support in partnership with the Spiritual Cultural Support Liaison Understand the cultural values of health, beliefs, and practices within the identity of Native American cultural groups and community and effectively serve as a communicator and liaison within multidisciplinary teams Understand the strengths and needs of our Native American communities and professionally navigate complex health and social systems effectively, including the perimeters of ICWA Qualifications Master's degree in social work with licensure in the State of Minnesota. LGSW required, LICSW preferred. Two years experience in a mental health care setting preferred. Knowledge of our Native American communities, urban and reservations, with the understanding of the vast tribal/intertribal differences. Experience working with Native American children, adolescents, and their families strongly preferred Credible role model in the Native American Community with established relationships with key partners, including community agencies and tribal entities Experience using electronic health records preferred Knowledge of Microsoft Office products including Word, Outlook, and Teams Ability to communicate in written and verbal form effectively and efficiently Ability to coordinate care with relevant staff Ability to shift between dissimilar tasks smoothly and rapidly Ability to make effective decisions while in stressful situations and circumstances PrairieCare is a psychiatric healthcare system experiencing rapid growth in the Twin Cities and southern Minnesota.
