Tasks/Duties/Responsibilities

70% Customer Services

  • Assess customer's needs pertaining to financial entitlements and legal services.
  • Assist customer with preparation of legal documents including living wills, advanced health care directives, guardianships, etc. ? Assist customer with obtaining appropriate entitlements including Social Security, Medicare, Medicaid, ADDP, etc.
  • Determine eligibility of customer for appropriate benefits and assist with completion of paperwork and accompany customer to meetings with providers, as necessary.
  • Research specific information pertaining to a customer's undocumented status, explains expected processes and facilitate needed documents when possible. Refer customer to attorney for further assistance.
  • Advocate against eviction. Help customer/tenant understand their lease and advise on proper steps towards measures the tenant wants to take. Link tenant with lawyer for further assistance.
  • Provide linkages with other service providers and/or volunteer attorneys to handle more complex legal needs.
  • Attend administrative law hearings and social security determinations with customers, as needed.
  • Conduct pro se clinics, as needed.
  • Develop and implement outreach activities including networking with other community- based organizations, support groups, religious organizations, etc.
  • Establish, maintain and update customer case records using approved agency forms and procedures. Ensure confidentiality of all records.
  • Prepare monthly reports in accordance with agency policies and procedures.
15% Reporting / Monitoring

  • Maintain and update client information in E-Compass and CHAMP
10% Hyacinth Representation / Advocacy
  • Attend and participate in staff meetings and supervisory conferences
  • Maintain working relationships with other community providers ?
  • Observe established administrative procedures
  • Provide written reports and maintain statistics as required
  • Represent the agency at community events, meetings, and conferences
  • Adhere to Hyacinth policies and procedures including client confidentiality provisions
5% Training and Certifications
  • Attend and participate in staff training opportunities
Requirements

Required Minimum Education: Bachelor's

Required Years of Related Experience: 2-4 years, a minimum of two years' experience in the field of HIV / AIDS

Required Knowledge, Skills and Abilities:
  • Experience with benefits counseling and client advocacy
  • Ability to do research quickly and effectively
  • Demonstrated ability to work with people from diverse backgrounds and interests.
  • Proficiency in verbal Spanish
  • Professional verbal and written communication skills in English
Preferred Minimum Education:
  • Bachelor
  • Preference may be given to candidates who have professional legal experience and have work with the target population.
Preferred Area of expertise:
  • Legal documents, benefits counseling, immigration
