The purpose of this position is to provide outreach services and a direct connection to immediate housing for people who are experiencing homelessness. This position will provide direct outreach and services to individuals identified as HIV+ by the public health street outreach team (Healthcare for the Homeless) conducting outreach in homeless encampments. They will work to identify potential residents to Clare s Hotel to Housing program and will directly assist with transporting and situating residents in the Hotel program. Through direct and collaborative assessments, this position will identify possible barriers in the client s life which may impact their ability to maintain permanent housing stability. This position will work to address those barriers and assist with procuring vital public benefits with the client to afford their potential housing placement. If barriers prevent a client from securing a traditional housing placement, this position will maintain a relationship to eventually to work towards placement in a no-barrier housing opportunity. The major challenge of this position is to provide direct services and support to individuals who often are living with multiple issues and diagnosis such as substance abuse, mental illness, traumatic brain injury and HIV. It may also be a challenge to maintain contact with the prospective resident prior to their coming into housing due to the transient nature of housing instability. This is a full-time position at 40 hours per week. Benefits include Medical, Dental, Vision, 4% match of 403B retirement fund, generous PTO and free access to 24/7 Tele Health and Counseling services. The Hotel to Housing Navigator will report to the Director of Supportive Services.
