In the Interim Case Manager role you will provide case management, appropriate referrals and linkages for clients that enable them to move to the next safe and stable housing environment.

Must be 25 years of age or older to apply, to comply with licensing.

Responsibilities:
  • Maintain a case management caseload by assisting clients in developing and implementing case plans to meet short- and long-term needs.
  • Competency and champion in Trauma Informed Care, Harm Reduction, Motivational Interviewing, Restorative Justice, LGBTIQIA competencies as best practices.
  • Ensure that all case management and continuing care documentation meet Night Ministry and all licensing and funding expectations.
  • Participate in program audits, representing ODS to auditors and auditing agencies alongside management.
  • Collect statistical and anecdotal information for grant supporting purposes and to evaluate program effectiveness of services.
  • Conduct weekly life skills groups with clients in program.
  • Significant daily milieu presence for client engagement.
  • Provide aftercare for former shelter clients in alignment with our grant expectations.
  • Connect clients to resources to improve client's stability, accompany clients to appointments when needed.
  • Enter data in ETO (Efforts to Outcomes), HMIS (Homeless Management Information Service), E-Cornerstone and complete exit assessments as required.
  • Participate in all required meetings All Staff, Youth Programs, dashboard learning sessions,
  • Conduct client case review as shift debriefing with program specialist and team meetings.
  • Other duties may be assigned.
Qualifications:
  • Bachelor's degree required in Social Services or related field, case management experience.
  • Creates an empathetic, trauma informed atmosphere.
  • Demonstrated experience working with at-risk youth.
  • Excellent interpersonal, verbal, and written skills
  • Ability to respond to emergencies quickly, calmly, and in compliance with Night Ministry policies and procedures.
  • Willingness to adapt schedule to meet programmatic needs, including some evening hours.
  • Position also includes providing life skills groups and interventions with youth.
  • Ability to provide screenings, crisis counseling and case coordination to families who receive CCBYS services.
  • Ability to accurately record client progress through case notes and reports in a timely manner.
  • Excellent conflict resolution skills.
  • Current driver's license; evidence of insurability.
  • Excellent interpersonal skills, written and verbal.
Essential Skills:
  • Spanish Language a plus.
  • Experience working with similar populations and in non-traditional patient care settings.

Work Environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time. The noise level in the work environment is usually moderate but may vary at the different locations required in this job.
