Founded in 2017, Covenant House Illinois (CHIL) provides critical protection and resources for youth ages 18-24 experiencing homelessness throughout Chicagoland and across Illinois. To date, CHIL has served nearly 2,000 young people, administering tailored, wraparound services, including interim and transitional shelter, workforce and education programming, clinical case management, life-skills training, and more to help youth permanently escape the cycle of homelessness. CHIL's programs and services are youth affirming, supportive, and rooted in harm reduction and trauma-sensitive care. With a newly renovated campus on Chicago’s west side and a recently enacted three-year strategic plan outlining a comprehensive vision of program expansion and service innovation, CHIL is uniquely positioned to meet the emerging needs of young people experiencing housing instability and provide the resources and care they need to achieve sustainable independence.

**Primary Responsibilities**:
The Case Manager (CM) provides direct physical and supportive services in a positive and nurturing manner to the young people served in Covenant House Illinois’ Youth Development Center and Residential Program using an integrated, multidisciplinary team approach. Reporting to CHIL’s Director of Clinical and Supportive Services, the CM will work with program Youth Engagement Staff and community partners to ensure the delivery of robust, comprehensive services to youth while ensuring implementation of agency philosophy, policy, and procedures.

**Essential Functions**:

- Ensure the highest quality of delivery of services to youth in accordance with the CHIL Principles, Mission and Core Values.
- Promote a safe and healing environment by utilizing the principles and practices of trauma-informed care, harm reduction and positive youth development in all aspects of work and youth engagement, including crisis prevention, intervention, and de-escalation.
- Manage a caseload of YDC and residential youth.
- Collaborate with youth in developing both short term and long-term goals as part of developing and implementing an Individual Service Plan (ISP).
- Ensure that youth have sufficient and proper ID (Birth Certificate, State ID, and Social Security Card, etc).
- Meet with youth on a regular basis and document interactions appropriately
- Ensure that progress notes are documented accurately in ETO
- Responsible for advocating on behalf of youth with outside service providers.
- Attend appointments and services when needed or requested by a youth.
- Have the ability and willingness to advocate for youth outside of Covenant House and in the community as need or requested.
- Ensure that youth’s files are compliant to all grant, government agencies and local, state and federal regulations.
- Assist with program audit and review preparation.
- Research and keep current external referral services.
- Work collaboratively with both internal and external service providers.
- Develop motivational tools for youth.
- Maintain healthy relationships with the youth, mindful of boundaries and mutual respect.
- Model supportive engagement with youth.
- Support program staff whenever possible and/or needed
- Participate in all required trainings, supervision and staff meetings.
- Facilitate groups on various life-skills issues, such as hygiene, problem solving, conflict resolution, self-esteem, and problem solving

- Understand and support the implementation of Trauma Informed Care, Harm Reduction, and Positive Youth Development and other best practices in youth work.

- Present as approachable, relatable, builds rapport well, and practices active listening with all individuals.
- Knowledge of community social service resources to assist youth with needs related to housing, employment, education, and social-emotional well-being.
- Knowledge of crisis intervention and ability to address conflict between youth; able to respond quickly and safely while calmly de-escalating the crisis situations and emergencies.
- Maintain confidentiality while understanding and complying with requirements of mandated reporting, HIPAA, and other limits to confidentiality.
- Willingness to utilize training opportunities in support of personal and professional growth, competence development, and to increase overall capacity to serve youth every day.

- Proficient with standard computer operating systems.
